有害化学物質取扱のWeb調査について
一度登録した後に変更を行った場合は、保健管理室へメールで氏名・学生番号・変更した旨をご連絡ください（hokekan@t.mail.nagoya-u.ac.jp）。
取り扱っている有害化学物質の種類によって、特殊健康診断の受診が必要・不要かが決まるためです。

Regarding the Web Survey on Handling Hazardous Chemicals
If you make any changes after registering, please notify the Health Administration Office by email with your name, student ID number, and details of the changes (hokekan@t.mail.nagoya-u.ac.jp).
This is because the necessity of undergoing a special health checkup depends on the types of hazardous chemicals you handle.
