保健管理室で健康相談をしたい方は以下のメモ用紙に状況をお書きいただき印刷して保健管理室にお持ちください。ない場合は保健管理室で記入していただくことがあります。
※メモが無くても相談は可能です。

If you would like to have a health consultation, please fill out the following notepaper, print it out, and bring it to the Health Service Center. If you do not have one, you may be asked to fill one out at the Health Administration Office in Nagoya University.
If you do not have a memo, you can still consult with the Health Administration Office.
