2026年度　特殊健康診断（有害化学物質取扱者）のお知らせ

令和6年4月から施行された労働安全衛生規則等により、危険有害性を示す化学物質等の取扱いについてリスクアセスメントを実施することが強化されました。（環境安全衛生管理室）

対象者

□過去1年以内の実験・実習で有害化学物質を取り扱っている学部生、大学院生及び研究生でＷeb調査を行い、特殊健康診断が必要と判断された方

□取り扱い物質はこちらを参照してください。

※調査時点で有害化学物質を取り扱っていない方は対象外です。

Web調査・予約

□保健管理室ホームページにてWeb調査を行う

Web調査期間：９月7日（月）10：００～　９月28日（月）10：００

保健管理室ホームページからWeb調査を行ってください。

※機構アカウントが必要です。ログイン後、「特殊（有害物質）」をクリックしてください。

※Web入力期間内に必ず入力を行ってください。

※特殊健康診断の受診が必要かどうかをWeb調査の入力内容で判断するため、入力期間終了後でも１０月５日（月）１０:００までに必ずご入力ください。

□Web調査で特殊健康診断対象者（必要）と判定された学生

Web調査入力後帳票の印刷が表示されるので、「学生特殊健康診断（有害）受検票」と「作業条件の簡易な調査」用紙をA4サイズでそれぞれ印刷してください。

※特殊健康診断後、「作業条件の簡易な調査」用紙の【担当教員確認欄】に担当教員の押印またはサインを受け取り、研究室または所属部局にて保管をお願いいたします。　保管期間について

□Web調査で特殊健康診断対象外（不要）と判定された学生　

帳票の印刷が表示されるので、「作業条件の簡易な調査」用紙をA4サイズで印刷してください。

その後、【担当教員確認欄】に担当教員の押印またはサインを受け取り、研究室または所属部局にて保管をお願いいたします。

保管期間について

日時・会場・持ち物

□日時：10月 ７日（水）、８日（木）　　両日ともに13:30～15:30　　（予約制）

□会場：保健管理室　1階入口よりお入りください

□持ち物：　

• 学生特殊健康診断（有害）受検票（A4用紙に印刷したもの）
• 作業条件の簡易な調査（A4用紙に印刷したもの）
• 学生証

注意事項

□「学生特殊健康診断（有害）受検票」と「作業条件の簡易な調査」用紙をお持ちでない場合は特殊健康診断を受けることができません。

スマートフォンなどのデジタル機器での健康診断に対応していないためご協力をお願いいたします。

□学生特殊健康診断を受診するには、定期（一般）健康診断の受診が必要です。定期（一般）健康診断を受診していない方は保健管理室ホームページをご確認ください。

□Undergraduate, graduate, and research students who have handled hazardous chemical substances in experiments or practical training within the past year, and who have been identified as requiring a special health checkup based on the results of a web-based survey. □For details on the substances handled, please refer to HERE（こちら）. *Individuals who are not handling hazardous chemical substances at the time of the survey are not eligible.

Web Survey

Period: Monday, September 7, 10:00 a.m. – Monday, September 28, 10:00 a.m.

Complete the survey on the Health Administration Center website. An institutional account is required. After logging in, select “Special (Hazardous Substances)”.

Input during the survey period is mandatory. If additional confirmation is needed, complete submission no later than October 5, 10:00 a.m.

Students Identified as Requiring the Examination

After submitting the web survey, print the following forms on A4 paper:

• Student Special Health Examination (Hazardous Substances) Examination Form
• Simple Survey of Working Conditions

After the examination, obtain the supervising faculty member’s signature or seal in the Faculty Confirmation Section of the Simple Survey of Working Conditions form and retain the document in your laboratory or department.

Examination Schedule

Dates: October 7 and October 8, 1:30 p.m. to 3:30 p.m. (appointment required)

Venue: Health Administration Office Please enter from the 1st floor entrance.

・Student Health Checkup Form for Hazardous Substances

*Please print on A4-size paper and bring it with you.

・Simple Survey on Working Conditions

 *Please fill in any blank sections yourself and bring the completed form.

・Student ID Card

Important Notes

• □Students who do not bring the printed Student Health Checkup Form for Hazardous Substances (A4 size) and the Simple Survey on Working Conditions form will not be able to undergo the special health checkup. Please note that digital devices such as smartphones are not accepted for the health checkup. We appreciate your cooperation.
•  □To undergo the Special Health Checkup for Students, you must first complete the Regular (General) Health Checkup. If you have not yet received the Regular (General) Health Checkup, please check the Health Administration Office website for details. → Notices → “Health Checkup for Students Engaged in Radiation Work or Handling Hazardous Chemicals Who Did Not Undergo the Regular Health Checkup in April”
