Application Forms etc. Related to Using Facilities
When hosting a tournament or event, holding a joint practice with a group outside the University that is high school age and below, or requesting to open the gates to the Yamanoue General Sports Field Area, you must submit an application form and obtain permission. When doing so, please submit the application documents to the Student Support Division Extracurricular Activities Section one month before the date of use, and reserve the sports facilities using the Sports Facilities Reservation System. Note that for group(s) of our University Department such as the Graduate School, you may submit the application form directly to the Health, Physical Fitness and Sports Center, only in the case where there is an official seal of the Department Head on the application form to hold a tournament or event (Form 1). However, there may be cases in which another group has already placed a reservation, so please submit the form as soon as possible.
