秋入学者健診/ Student health checkup (FY2025 autumn admission)

定期健康診断は学生の健康保持・増進等を目的として学校保健安全法に基づき実施し全学生が受診する必要があります。
その年度の定期健診を受診した人は健康診断証明書発行のサービスを受けられます。証明書の期限は受診年度内となっていますので健康診断は毎年受診してください。
The periodical health checkup is required to be taken by all students as prescribed in the School Health and Safety Act, with the aim of health maintenance and health promotion.
Students who have taken the periodical health checkup for this academic year are able to have a health checkup certificate issued to them. The certificate is effective until the end of the academic year, so please take a health checkup every year.
対象者：2025年秋入学生（4月に籍のある者除く）
Eligible person : Students entering fall 2025 (excluding those enrolled in April)

1. 日程 / Date
２０２５年１０月２１日（火）２２日（水）
October 21st Tuesday and 22nd Wednesday, 2025
予約期間は、１０月３日（金）からです。予約可能な人数には、上限枠を設定しています。
予約は先着順となります。なるべく早く予約をしてください。
Appointments can be made starting October 3rd. There is a limit to the number of appointments that can be made per day.
Appointments are made on a first come first served basis. Please make an appointment as soon as possible.
２．当日必ず持参するもの /Items you need to bring with you
健康診断に必要な検尿容器は所属事務室より配布されます。配布方法などは所属事務室にご確認ください。
Urine collection tubes are provided by your own Faculty.
①健診受付票（印刷した用紙またはスマホ等の画面表示）
※忘れた場合，受付できません。
1.Your health checkup registration form (printed or on your smartphone):
*You will not be allowed inside without your registration form.
②尿
できるだけ朝一番の尿を持って来てください。
女子で生理中の人は採尿せずに受付へ申し出てください。
2.Urine sample
– Use the first urine of the day. Be sure to put your name on the tube.
– If you are having your period, do not take a urine sample and tell the reception.

③両面無地のＴシャツ（白以外も可）
※刺繍、リボン、ポケット、ボタン、金属、プラスチック（留め金）、湿布、カップ付 などの飾りのないもの。
X線撮影の注意事項 必ず確認ください【未受検注意】
※Ｔシャツの上に羽織るもの（前開き）着用可能です。
3. For those who will undergo a chest X-ray：Plain T-shirt
＊T-shirt must not have embroidery, ribbons, pockets, buttons, metal, etc.

３．注意事項 / Reminder
健診に影響する可能性がありますので、健診前日の暴飲・暴食・激しい運動は避けてください。
健診当日、貴重品は自己管理になります。責任を負いません。持参する荷物は少なくしてください。 軽装でお願いします（ワンピース・ブーツ･アクセサリーなどは避けてください）。また、髪が肩から下におりないようヘアゴムなどでアップスタイルにできるようにしてください。
LGBTなどの方で、男女別あるいは集団での健康診断に不安な方は、遠慮なく、保健管理室のメール（hokekan@t.mail.nagoya-u.ac.jp）でご相談ください。
Please avoid excessive drinking, excessive eating, and/or strenuous exercise on the day before the examination, as these may affect the results of the examination.
On the day of the examination, you are responsible for your own valuables. Dress lightly and bring the least amount of possessions possible. (Please avoid wearing a one piece dress, boots, accessories, etc.) Moreover, please make sure to put your hair up with a hair tie, etc. so that your hair does not hang down the shoulders during the chest x-ray examination.
Health Administration office respects LGBT people or sexual minority. If you are worried about group medical examination segregated by sex, please do not hesitate to come to our office or contact us by E-mail (hokekan@t.mail.nagoya-u.ac.jp). We will reschedule for another.
４．Web予約について / Reservation of WEB
PC,スマホから予約入力できます。
①健康調査票を入力終了後、予約が可能となる。
②予約完了後に【健診受付票】を印刷する。（印刷はＡ4用紙で行ってください。写真は不可。）または健診日当日、スマホ等で画面表示ができるようにしてください。
※予約に機構アカウントとパスワードが必要です。
機構アカウントが無い方は取得してください。取得には時間がかかりますので、早めに所属事務室に確認してください。
※予約健診日に受診できないことが分かった時点でキャンセルし、改めて予約してください。
※学部1年生の方へ
【感染症】のWEB入力は、12月末まで可能です。

On the health checkup day, please bring a downloaded copy of the form on your smartphone or other devises, which you should download after making your reservation.
(Please print out in a A4 paper. We do not accept a photo.)

However, if you do not have one, please make an appointment after receiving your.
※Please promptly cancel and reschedule your appointment if you discover that you are unable to take an examination on your scheduled day.
※To the first-year undergraduate students, please finish entering your [vaccination record] on the web by the end of December.

※休日は健診についてのお問い合わせをいただいても回答ができません。早めに予約や準備をしてください。
※予約時間が選べない方は、受付票をダウンロードし受付時間内にお越しください。
※放射線や有害物質を扱う実験・実習をする人は、特殊健康診断対象者です。定期健康診断も受診してください。

We will not be able to respond to inquiries on health checkups during the holidays. Please book your appointment and make necessary arrangements well in advance.
If you are not able to book an appointment for a specific time, please come in with your registration form during our hours of operation.
If you conduct experiment or practical training with radioactive/hazardous materials, you must are subject to special health checkup. You also need to take the periodical health checkup.

Q 定期健康診断を受けなかったらどうなるか？
A 受診しなかった場合、奨学金受給や就職活動に必要な健康診断書を証明書自動発行機で発行することができません。定期健康診断で何らかの異常が見つかった場合、再検査や精密検査または医療機関を紹介しています。ご自身の健康管理のために健康診断を受けましょう。
Q. What will happen if I do not take the periodical health checkup?
A. If you fail to take the examination, for activities that require a health certificate, such as receiving scholarship or job hunting, a medical certificate will not be able to be processed and printed from the certificate issuing machine. Following the health checkup, if any irregularities have been discovered, a reexamination or more detailed examination as well as being referred to an outside medical institution will occur. So be sure to undergo a health checkup for the sake of your own health.
